I created a new model release form. However, when reviewing the submitted form the only thing I can see is the name and signature. How can I view/save the entire executed agreement?

Are you viewing the form submission in the Submissions page? If yes, then you just need to enable the "Show Headers and Texts" option. Here's a guide: https://www.jotform.com/help/305-How-to-Show-Headers-and-Text-in-the-PDF.
Please take note that in the Email Notification or Autoresponder Email, the "Text" tool is not included in the email content. You need to manually put in the email content using the email editor.
Alternatively, you can enable the "PDF attachment" option in the email alert. In this way, a PDF submission will be attached in the email alert. You can do this in the "Advanced" tab of the alert.
After doing that, you need to enable the Show Headers and Texts option on the Submissions page. In this way, the headers and text fields will be included.
