Why all my forms are disabled?

    Hello,

    Why My old form and i just make new form but same, Form not Found...

    please give me information,

    Hello sugengw07,

    Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Premium Akun" has fields like "Username & Password" which makes it suspected of being used for phishing.

    Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password field and activated your account again. Do try to sign in to your JotForm account and get back to us if you have any trouble.

    Kindly refrain from creating form with email/password, login/password or credit card details.
