Error message: Form not found. This form is disabled.

I've published my form and when I try to look at it or preview it, I get a 'Form not found' error and it says that my form is disabled.  I have it enabled...not sure where else to look.

Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Traveler Information" has several fields like "Password" which makes it suspected of being used for phishing.

Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password field and activated your account again. Your form is now back online and should appear correctly in your website.

Kindly refrain from creating form with email/password, login/password or credit card details.
