How to upload form

Hi, I'm a web designer and my client would like leads to upload their credit reports on his website. Kind of like your "Upload a Screenshot" below with the button that says "choose file." Is your app capable of accomplishing this? If so, where does the form they upload go and how do I install it on the website? Thanks!

Once you finish building your first form, simply grab your form's Embed code, and embed it in your client's site: https://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page 

Note: make sure to register a free account first before building your form, otherwise it will be created in guest account which relies on your browsers cache.
