My account got suspended

My account is suspended. I created a payment authorization form and I just saw another user with same problem. feedback said not authorized by terms of use. I didn't know. I am happy to integrate a Paypal button on my site. I do have other forms that do not collect any such information. Can you please reactivate my account so I can rectify issue and keep using other forms?
