Not all fields are showing up in the form email

When an application is filled out the name is not showing on the form r-mailed to me. I checked to make sure it is showing on the form in jot Form and it is. Although it is blank on my copy

I would suggest re-creating your form email template. Just delete the existing template and create a new one.

It's not the form that you need to re-create, it's the notification email. Please check the guides that I've shared.
