I get a message that says form not found when I published the form

When I publish a form and click on the link, I get the message "form not found."  I have tried cloning the form and editing an old form.  Nothing works. Please advise. I have asked this question and have so far,  received no response. 

One of your form with an unusual term has caused our system to auto suspend your account. I have evaluated the form and I found nothing that violates any part of our terms, so I have reactivated your account. Please check again.
