This form is visible on desktop browsers but not on mobile or ipad. It has been functional for the last 6 months, but started this error today. Please advise.Page URL:

I checked your form on mobile and all images are showing as far as I can tell:
Are they still not showing on your end? If not, let us know what device/browser you are using and we will test again.

Still not showing on our end. It's on an updated iPad with native browser. It's functional on my mobile device (android) now, but still not on ipad.

I checked the form on an Ipad mini and the images showed on that as well:
Is the ipad on a different network than the android device? If so, it is possible something on your network is blocking the resources.

This is absolutely what is happening - resources are being blocked inexplicably by our firewall. No changes have been made manually, but very likely an automatic update to our firewall is causing this. Has this been an issue in the past and, if so, what was the solution?

I do not believe there were any updates on our end. If you check the web browser console you should be able to see what specifically is being blocked. Usually this can be brought up by pressing f12 in the browser.
You may need to update your firewall settings to allow through what is being blocked or contact whoever manages it for your organization.
