Hello,
I received an email that my forms have been de-activated. When I went to check out my account, it seemed as if someone sent around 200 spam form submissions.
What can I do to re-activate my account without having to upgrade, as the form submissions were not actually submissions through our form, but spam?
Thanks,

I have added the IP address of the submitter to our ban list and then reactivated your account. I would suggest doing the following to prevent multiple submissions/spams in the form:
1. Add a captcha field in the form. You can use our Google re-captcha widget:
https://widgets.jotform.com/widget/google_recaptcha
https://widgets.jotform.com/widget/google_invisible_recaptcha
Here's our guide to add a widget in the form: https://www.jotform.com/help/252-How-to-Add-a-Widget-to-your-Form.
2. Set a unique limit in the form. Point the unique limit to the name since the spammer is using the same name.

Note that I reset your limit back to zero. Just delete the spams in the form.
