If the correct answers are selected and form saved, the quiz only works once then the answer indicators return to blank. If the answers are added again, VIEW QUIZ RESULTS will show past entries correctly graded and cycle begins again. I've tried clearing my cache, different browsers (IE 11 and Chrome). Also, a "t" is added to the correct answer on the feedback page. How can this be resolved?Page URL:

As I can understand the issue you were experiencing, you set up your correct answers on the Quiz app, but it only worked once, then when you went back to the Quiz app you found the answers cleared up, so your selected answers were blank.
I can also see the issue you're mentioning about the "t" added right before the correct answer, when an incorrect has been selected. I have tested your form and I can see such issue:
