"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."
Asking for credit card information is not allowed, therefor, your account will remain suspended.

Hmm. I was not aware it couldn't be used for this purpose. I even purchased & implemented SSL on my website (https://www.bmclimo.com/) to protect cardholder data. In addition, there are options to add credit card fields to the Jotform -- so this never came to mind. Do I have to use pre-included payments options instead?
I removed the CC info from the form, can my account be reactivated please?

Thank you for understanding and we appreciate your cooperation. I have checked your form and see that the guilty fields are now removed. You may use any of the payment integrations available in the form builder in order to receive the payments. These payment methods comply with the PCI/DSS security standards and the payment information should be safe and encrypted with the payment processors.
