My form has stopped working

    I have checked all settings and everything is on enabled with no limits and my form is still not working.   I have a paid account and still not working.  I've clones it to see if there would be a difference and there is none.  Can someone please give me some idea why? 

     

    screen says:  FORM NOT FOUND

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I have checked your forms and see that they are collecting credit card numbers using plain textbox fields on the form. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing. 

    In order to activate your account, it is required to agree to our terms and remove the guilty fields on the form. Please confirm so that we can delete the guilty fields on the forms and activate your account.

    I USED YOUR DIRECTIONS FOR THE FORMS TO BE ENCRYPTED.  I HAVE A COPY OF THE FORMS AND THEY ARE. I WILL REMOVE THE FIELDS, BUT CAN I USE A LINK TO AN ALTERNATE ADOBE E-FORM FOR AN AUTHORIZATION FORM OR NO?

    I HAVE TAKEN OFF THE REQUESTED FIELDS AND IT STILL ISN'T WORKING.  PLEASE TAKE A LOOK AND LET ME KNOW.

    Hi,

    I checked your forms. I would like to give you some more information about sensitive fields in Jotform. Depending on our Terms of Use, sensitive data can not be collected, even through 3rd party via Jotform. You can only use our payment tools in order to get payments. If you don't want to get payment and if you need only payment authorization you can enable the "Authorization Only" feature.

    Also, another the reason that your account was disabled and suspend was due to the passport number and passport expiration fields on the form. We do not allow for the collection of sensitive personal information such as credit card details, social security numbers (SSN), or passport IDs.

    I see that your cloned form which is titled as "Clone of SHAWN + KENYA'S BACHELORETTE RETREAT" has still some phishing fields. 

    Please delete these all sensitive fields from all of your forms. 

    Also, I would like to inform you that I have just refunded your money which is for our bronze plan.

    I have inspected your form and I saw that you have deleted all fields that were violating of terms of use. I have activated your account.

    Please do not create similar forms or your account will be suspended again and we will not be able to activate it.
