I'm trying to preview a form I created

Hi,  I'm trying to preview a form I created, however it states that my form is not enabled, however it says that my form is ready to be viewed.  Can anyone help?

I have checked and your account is autosuspended by our phishing system. Because of that your form is disabled. Your Primary EX Line Order Form has username and password fields and it is against our Terms of Use to collect login credentials.

I have deleted mentioned fields from you form and I have activated your account. 

Please do not create similar forms.
