Why the answers are duplicating on selection?

I am trying to create a narrative from patient answers. I am using the TEXT box with fields to put answers in to sentance forms. 
I created conditions for two similar questions that will populate the text but when one " worse by" is selected, it keeps repeating the answer. Please tell me why.

The problem there is that you set in the condition that if the field is filled copy the text. So for any selection, the text will be copied and therefore duplicated.

Maybe a better would be to use Text field for such purpose:

And then simply show it when Worseby_1 is filled:

But this might better work with dropdown fields.

Could you please tell us more a about the logic of the condition, should the text show for each selection?

And is the text for each: "The complaint is aggravated by"?
