We currently don't have a tool that will convert an Excel file to a JotForm form. However, you can build the form from scratch using our Form Builder. Here's a guide: https://www.jotform.com/help/2-How-to-create-your-first-web-form

You can create a 4-column form by using the Shrink function and by increasing the width of the form. Please check these guides:

    https://www.jotform.com/help/90-Form-Field-Positioning
    https://www.jotform.com/help/328-How-to-Position-Fields-in-JotForm

For the "Team Member" table, you can use the "Input Table".

Hope that helps. If you have any questions or if you need help, let us know. Thank you.
