How do I edit the existing form?

I would like to customize our BR Jot Form but don't know how to that.

I might need a specific login to be able to do this

Hello Jan,

Yes you need to login to your JotForm account to be able to edit forms. I did check the form URL which you have mentioned and found that it is associated with account "BespokeRide". The profile email address of this account is info@bespoke-ride.com 

Do you have access to this account or email address? If yes, you can reset your account password by clicking on the "Forgot Password" link which is in "Sign In" window. You can use your username or profile email address to initiate the password recovery. Please check the screenshot below:
