Jot from not allowing files to be attached

My form at www.safetynetwireless.com/agent/

is not allowing photos to be attached before submitting .

Then giving an error message that says "this filed is required'

I cloned your form to test it and it seems to be working on my end.  Try replacing your current embed code with your forms iframe code:

Usually when the form itself is working but the embedded form is not, this resolves the issue.
