Usually this happens when accounts have been signed up with a partner organization (e.g., a hosting provider) and the provider's DNS fails.

I have forwarded the issue regarding the CloudFlare error when accessing your form with custom domain name http://forms.martinsinc.com//form/41495484116155
Since I could not reproduce the problem here, kindly check it again this time. If the problem persists, let us know so we can add it to the report ticket.

The issue is related to CloudFlare's end. We have contacted them in order to solve the issue.
