I received reports that our form isn't working. I just tried to open it in Google Chrome and Firefox and I got the following messages (see screenshot).
Can you please help me fix the issue ASAP? Demand is very high right now for this form, so your help is greatly appreciated!

Thanks for reporting. I was able to reproduce this problem. I think something is breaking the form script.
I have now raised this to our developers for a resolution. Once this is resolved, you will be informed here.

This problem is now fixed. Your form is working fine now.
