Why is my form disabled?

    I see that the account is suspended because of the form titled "Effortless Travel/Outside Agents Consent Form for 1 Traveler". Unfortunately, It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.   

    It is required to accept our terms of usage and agree to delete the guilty fields on the form in order to activate your account. Please confirm so that we can activate your account.
