Why is my login suspended? I changed my password. Still says suspended.

My form has been disabled as well. 

Your account with username piersatisfaction is suspended, because the forms your created are violating our Terms of Use. Your forms are used to collect credit card info with plain text fields and that is not allowed.

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

In order to activate your account, it is required to agree to our terms and remove the guilty fields on the forms. Please confirm so that we can delete the guilty fields and activate your account.

I confirm you may delete the guilty fields. Will you show me or direct me to where I can use the correct field for Credit Card paymemts please?

I have deleted credit card fields from your forms and I have activated your account. 

Please note that card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

If you want to have payment fields inside of your form you have to use one of JotForm payment tools.

If you want to charge your users manually at later date, I would suggest you to use on of payment tools that has built in credit card fields like Paypal Pro, Stripe, Authorize.net.
