I am locked out of my account

    When I try to login to my account it says "Log-in failed - SUSPENDED"

    One of your forms has been created to collect credit card information using our plain text fields. Please note that we do not allow such form. We consider it as phishing which is against our Terms of Use (http://www.jotform.com/terms/). I have reactivated your account, but please refrain from adding the same fields again.

    If you want to collect a payment at a later date, you may consider using our payment integration such as Paypal and enable payment authorization.
