All of my forms are disabled????

All of my forms say "FORM NOT FOUND - DISABLED", I need this fixed!

I had to disconnect the form from my website, as it's how clients contact me, so I don't have a web address to put in the box below.  My website is:

I have reviewed your account and I can see there is one of your forms where you're asking for credit card details through plain text fields, this is why your account has been suspended, do note that asking for credit card details through plain text fields goes against our terms, I would suggest you to read them in order to ask for such information again and get your account suspended: http://www.jotform.com/terms/
