How do I load a WMV video to my form?

I have a WMV video that I would like to add to my form, how do I do this please? Cheers

Do you want to play your WMV video in form? 

Please note that it is not possible to play video form your local computer. Here are the steps I would suggest to achieve your requirement:

#1. Upload your WMV video to Vimeo. The following guide should help you how to upload WMV video to Vimeo site:  https://join.vimeo.com/uploading/

#2. Grab the URL of the video which you have uploaded in Vimeo.

#3. Add "Vimeo" widget in your form and add the video URL in the widget.

Alternatively, you cal also add your video in YouTube and grab the YouTube video URL which can be added in the "YouTube" widget on form.
