    my form is being cut off with the width where I am using a widget.  It runs into the border of my site and can not be seen well.  Is there a way to fix this.

    Some of the widget needs to be adjusted.  If you add the following CSS to the widget, it should decrease the width enough to fit the page:

    td.col4 select, td.col5 select {
    width:200px;
    }

    th.col3{
    width: 50px !important;
    }

    th.col1{
    width: 50px !important;
    }

    td.col1 input{
    width : 100px;
    }

    Here is how the form would look with the code added:

    I checked your page and everything fits just fine now:

    Try clearing your browsers cache/cookies to ensure you are loading an updated copy of the form.

    Yes, I had to change the CSS to: width:160px

    Then it fit no problem.  Thanks.

    Is there a way to make the text smaller for "Name of Ensemble, Directors Name, Number, Classification, and Adj Division?

    Yes, it is possible to change the font size for that text, please inject this CSS code to your widget following the instructions given above by my colleague:

    th {

        font-size: 10px;

    }

    You could change the font size value if you want so you get a smaller or greater font.

    Hope this helps.
