I checked your form but I wasn't able to replicate the button turning blue on hover, is that happening on some particular Browser?
Also, I tested on a mobile phone and the form looked fine, on which device you were testing?

It happens in IE, Firefox and Chrome.
Using a 15" laptop. -

I tested this issue in Chrome, IE11, FireFox, and Edge on Windows machine and Chrome, FireFox, and Safari on Mac, but I wasn't able to replicate any issue.
Could you please take a screenshot of what you see and post it here, here is how:

Also, are you testing the direct link of the form or is the form embedded on the website?

Hello,
I have tested your form on Google Chrome, Mozilla Firefox and I couldn't replicate the problem about being responsiveness as you can see the screenshot below:
Did you mention about the images on the top? Because this is the normal behavior of texts as they created in 2 columns.
Moreover, I couldn't see the behavior of the submit button when hovering over it as you can see below:

You have just replicated the issue I'm encountering regarding the responsiveness.
Why is it that I get to see the page 100% correct on a smartphone where as on a desktop the page gets squeezed into 2 small columns instead of the form being displayed under the text as it should? -

To my understanding, you want the form fields under the text field of the form. If yes, I cloned your form and I noticed that everything has the shrink feature enabled. That's the reason why the form has two columns. To fix that, I have disabled the shrink feature of all the fields. Here's the clone form URL: https://form.jotformpro.com/71594128077967
