See my bookkeeping form. the widget no longer works. It was fine in the free jot form account, upgraded and now it doesn't work. I created a new form to see if the form was corrupted somehow and it still isn't working. I tried from multiple devices, still not working. Rebooted devices, still not working, cleared cache, still not working.

I have cloned your form and Take Photo widget is working on my form. I have also created new form and it is working there as well. Here is my cloned form: https://form.jotformpro.com/71586027887975
I have clear your form cache. Please update the widgets on your forms to see if that will resolve your issue.
