When I receive the form by email, It is also forwarded to my phone via text and there are a lot of question marks at the beginning and at the end. The screen shot on top is the one that comes to my email, the bottom shot is what it looks like when it arrives as a text.Page URL:

May I know how is it that you are sending the notification to your phone via SMS? Please note that those characters may appear, because HTML is not supported in SMS.

You can edit the notification email content and make it more simplified. For example, you can delete the table from the email content and use just texts as seen below:
