Why 2 of my forms are disabled on the website?

    Hello, I do not understand why 2 of my forms are disabled on the website, but they live on my account. 

    Site is www.newyorkcity-limo.com

    Online Reservation and Contact us 

    Form not found
    This form is disabled.
    I did not disable them!

    I have checked and your account is suspended. Two of your forms are used to collect credit card info on plain text fields and that is against our Terms of use

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    In order to activate your account, it is required to agree to our terms and remove the guilty fields on the forms. Please confirm so that we can delete the guilty fields and activate your account.
