Not able to view entire form on iPad

I use go daddy website tonight for our website www.l3tennis.com. I have imported the go daddy form for our registrations, tennis camp 2017.

It was working, but within the past week, I realized that it is not working on ipads- it shows up, but will not allow a scroll down to view and fill out the entire form. That was happening on android mobile, bit I fixed that by making imported space on the web page larger. I have contacted go daddy, the host, and they said that is it something with the script from jot form.

It works fine on desktop version.

We have had this as form for a few years, this is a clone, as you will see. This was not a problem prior.

I have opened your form on the iPad2 however, I did not encounter any issue on my side. The form is allowing the scroll down and could submittable. As you can see below screenshot fist and last part of the form:
