form not found error message

I have one form I have been using for some weeks, but now when I try to preview or link to it we get "form not found" error message. The form is visible in the My Forms section, along with it's prior submissions. I am also not able to embed the form into my web page, which also worked before. The embed code shows as regular text on the page.
