Why the whole form is not shown anymore as it did before?

    Hi,

    your form worked well even on mobile devices but now for no known reason most of the form is missing. Works great on desktop browsers but misses much on mobile ones.

    Do you have any idea why?

    I was able to replicate the issue on my desktop PC when I adjusted the browser window. Please try to re-embed the form using the iFrame method. Here's a guide: Getting-the-form-iFrame-code.

    Also, I recommend you add the Mobile Responsive widget to your form. Here's a guide on How-to-Add-a-Widget-to-your-Form.
