We can change your username on our side. Please provide us with the new username that you would like to use, it should be available for us to make the change, a good way to know, if the username is available by checking it on the sign-up page: https://www.jotform.com/signup
Type the username that you would like to use and it should tell you if this is either available or not.
You can change your JotForm account email in your My Accounts Profile page
You can go to your account page via this link: http://www.jotform.com/myaccount/

Your username has been changed from Wayneacct5 to KHSalumni as requested. Just logout then log back in using your new username to confirm.
Just to add, in case you're using Custom URLs, your previous URLs will stop working. Should you wish to set it up with your new username, set them up again within the Form Builder.

If you're not using Custom URLs, then you may ignore this part.
