I checked you account and I see your form was disabled because it was requesting credit card info. This is not allowed because it violates our terms and conditons:

However I visited your site and I can see its an honest mistake. So in this case here is what you must do:
1. Log into your jotform account and remove the credit card requesting fields.
2. Use one of our payment processors
