My forms have been disabled

When I click "view forms", it says page not found and that my forms have been disabled. Why is that?

I see that your account is suspended due to the form Clinical Supervision Billing Form that is collecting credit card information using plain text fields. Unfortunately, it is not allowed to collect credit card information on plain text fields using JotForm. Such forms are considered as phishing forms and associated accounts will be automatically suspended. Please refer to our terms of usage for more information on phishing. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.  

It is required to agree to our terms of usage and to delete the guilty fields on the form in order to activate the account. Please confirm so that we can proceed further.
