This is not a common issue. I have tried to reproduce it, but I was not able to do so.
Could you please re-integrate your form with Dropbox to see if that makes any difference?

by filling out the form 7 times and uploading a doc each time. 4 of the
docs made it to the drop box. The other three were you your servers. If
it happens again, I will shoot you an email.
