Why are all my Forms down?

Our Phishing detector have detected an existing form in your account that was flagged as a phishing form.
Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose : sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
You should delete the form and remove it from your account first before we can request for consideration to reactivate your account and other forms.

That is not a Username for Login, its a Username for a Apply Form.
We Generate a Username an a APPLY-ID with our Website.

I have Renamed the Field.

I checked the form and I can see that it is now renamed to a different word. Upon checking it further, I have noticed the word "Hacker" to your form. I have removed it and activated your account back in good status so you can manage your forms properly.
