How to issue a certificate upon completion of a form?

    How can multiple participants get a pre-formatted certificate that contain certain information fields that the person completing the form has filled. This is only issued to the person completing the form.

    The participant would then be given the link to complete a form that includes the following fields for example.

    Name, license number and number of credits obtained. The fields would be contained in the demographics provided by the individual.

    To achieve this, you must display the certificate in your form's thank you page. You will be able to customize the look of your thank you message as well.

    You will have to submit the form and then see the certificate in the thank you message page. There are lot of scope to further improve it as far as the layout is concerned and that can be looked into.

    As far as the URL and the date is concerned which is being displayed in the printout, that is actually part of the print setting and can be easily removed from the printer setting.

    Alternative way is to create the actual certificate in the HTML and then add it in the thank you message. You can also create your own thank you page and redirect your user to the certificate page after the form is submitted. You will have to read the post data in your custom thank you page.     In addition to the example provided by the other user in the second post of this thread, you may also consider using a PDF report to customize the submission. You may customize the PDF report as per your requirement from the submissions view.

    You can also attach the PDF report to the email auto-responder so that the user can receive it.
