How to issue a certificate upon completion of a form?

How can multiple participants get a pre-formatted certificate template that contain certain information fields that the person completing the form has filled. This is only issued to the person completing the form.

The participant would then be given the link to complete a form that includes the following fields for example.

Name, license number and number of credits obtained. The fields would be contained in the demographics provided by the individual.

In addition to the example provided by the other user in the second post of this thread, you may also consider using a PDF report to customize the submission. You may customize the PDF report as per your requirement from the submissions view.

You can also attach the PDF report to the email auto-responder so that the user can receive it.
