Data cannot be imported into the form from external sources.  Forms could definitely be used for the survey, but users would need to enter their own data.  You could create pre-populated links for the form:

https://www.jotform.com/help/71-Prepopulating-fields-to-your-JotForm-via-URL-parameters

But each link would need to be created individually.
