I currently have a form on a Wordpress site and it is not loading unless the page is refreshed. How do I correct this?

I was not able to see the form in the Page URL you provided. Are you referring to the "Submit Your Music" page?

Yes....thats exactly what I was referring to....if you go to that page the
form does not show up unless you bit the refresh button
...
