How can I change my account username?

How can I change the username of my account?  I'm transferring it my church, since all of the forms are for them anyways.  I just want to be able to create my own account now.

Or can I transfer forms if they create an account?

Please note that we can assist you with both options:

1. If you would like to change your account username, all we need is three (3) username suggestions by the order of your preference, just in case your first pick is already taken, and we will then change the account username ASAP.
