On the form I created is a authorization page, that allows the company to run a background check for a driver. The problem I am running across is even though the state allows electronic signatures, they require the printout to be laid out exactly like their form (2 pages). Is there a way to do such a print out?
See page 8 of my current form, currently only the question on authorization prints along with the signature below that. I need to print everything from the PSP line down.

I tested your form and the text is showing on my print:
Is the issue that you want that text on some other pages?
Or you can not print it from the submissions?
Please let us know so we can have another look.

a screen I can not find on our account. The left side of the screen you
show has the print option I am looking for to print the contents of the
form, and not just the answers. Can you tell me how you got to that
screen, so I can check it out?

The screen my colleague is showing is printing directly from the form in the browser. This screen can be different depending on what browser you are using but in most browsers the print function is readily available from the options menu.

If you are trying to print from a PDF or the submission page, while viewing your submissions, there is an option to include headers and text in your submissions:
