Sorry for all the troubles here. Can you try to clear your forms cache? Here's a guide how to do it.
If that doesn't work for you, then try to re-save your form.
Let us know with your progress on this one.

Thanks for the response. I cleared the cache on Jotform as well as my browser and still getting the same error. Followed all the directions in the link as well as resaving the form.

I have cloned your form and I was able to replicate the issue. I am also getting the Form Failed error on my end:
To fix the issue, please delete the Signature widget and replace it with a new one. Please check the screen animation guide and result:
