I checked your form and did not see any form. Can you elaborate more on your concern, so that we will be able to advise you accordingly?

I'm having issues trying to replicate or picture out your issue. Is it possible if you can provide us a screenshot? So that we will be able to advise you accordingly.
I tried integrating MailChimp on my test account, and it works fine. I entered first my API key, which I can retrieve at MailChimp's end. (Here's a guide on how to generate or get your API key http://kb.mailchimp.com/integrations/api-integrations/about-api-keys)
From there, it will ask for your MailChimp list. Make sure you have created a list at MailChimp's end.
Then, match the fields in your Mailchimp list to your JotForm fields.
