Jotform embedded javascript code not functioning properly

I embedded  javascript jotform code but it id truncating the form and not fully displaying. The last element id the submit button which is not displaying using the embedded code,

the code is as follows:

The jotform displays properly if I use a browser and go to the form link.

I tried the embedded code code on another page with the same result

The site also uess 2 other jotforms with embedded javascript  code and they work properly.

The site/pages are  HTML5
