Why my form is not showing properly?

    Good day, we have been using your site as form for our transactions in our business, yet we are currently experiencing failure in the form. which is one of our branch they cannot open the form with the link i gave to them.

    Some of my forms when they opened displaced lack of buttons, it shows incomplete. 

    Every time i check in the page its totally in-tack.

    hello sir,

    it seems to be working fine when i browse the link you gave us through google chrome, everything was alligned and works perfectly. Would you care to elaborate your problem a little for further assistance.

    We have not received similar reports from the other users regarding the issue you are encountering. I checked the form link you have provided (http://form.jotform.me/form/42400749206449) and it seems there is no problem opening the form on my end. 

    May we know which device, version, and in what browser you have encountered the issue? We recommend users to use Firefox or Chrome to access the forms. Please also make sure that you have provided them the correct form link.

    As per checking further the form, I noticed that there are some conditions that were set up on the form to show and hide the fields. Perhaps this is what you are referring to?

    Good day,
    They are using chrome, in windows 10.
    i am using the latest form layout.
    and why my submission over runs
    it suppose to refresh after 1 month right?

    I tested your form in Chrome on Windows 10, but I wasn't able to replicate this issue, the form is showing fine.

    Could you please let us know if your users are accessing your form by using the direct link (http://form.jotform.me/form/42400749206449 ) or you embedded it in a website?

    Also, is this issue happening with users that are using the same network, since it could be a network issue?

    Were you able to replicate this issue?
