Ok, so I wan't to know how AdvertisingFree4u.co.cc made there form. I know they are using you guys, JotForm. He/she made it like you submit a your url and other stuff, then it gets your URL on the sidebar. I would like to know how to do that. Also what is that form on the sidebar? AdvertisingFree4u is using Webs.com.

You are correct, it does seem that the website is using JotForm.
A quick view of JotForm FAQ can provide you info about JotForm forms.
As for that 'sidebar', it was a actually a Feedback Form. Here is a guide How-to-create-a-Feedback-Button-in-minutes .
If you haven't done so, you may want to create your JotForm account here. Using only a guest account can have a lot of limitations on your end.

Hello SpottyFountain8. I'm assuming that you're referring to the Your Website side bar. What they did was just actually showing the HTML report on the sidebar. So the report submission appears on the right panel after you make a submission.
You can read on this user guide on how to create a Grid Report (Quite similar to HTML report) on this user guide.

Sorry for any confusion the replies may have caused =) It was not intended.
But Liyam did provided the right answer on the HTML URL report/result that are displayed in the right-portion.
I was describing on the other hand, the Contact Us 'sidebar' feedback button.
