How can I create multiple colums on my form?

    I'm trying to add a table or drop down options for people to put in a piece of equipment and the quantity they want picked up and delivered.  We are delivering picnic equipment to a park for a major picnic here in the Chicagoland area.  We want potential customers to fill out the form and type in the equipment and quantity.  For example:

     

    Equipment                             Quantity

    Tent                                        1

    Coolers                                    3

    You could use the input table field, it will allow you to display multiple fields in a table, it can be found under the quick elements fields:
