Why am I getting error messages on my forms?

I'm getting error messages on my forms, I've checked they are Published and Enabled.

Your account is suspended due that you are requesting password in plain text fields on your forms "Andy Beattie Photography - Booking Form (Card Payment)"  and "Andy Beattie Photography - Booking Form (Paypal)".

Gathering this type of data in plain text fields will violate our Terms of use, as this is only allowed through encryption along database administration; and our forms do not provide this type of method.

We have removed the guilty fields on this form and reactivate the account. You must refrain from doing a new form like that to prevent a potential suspension of your account.
