How to select several options?

I create a form with several questions including several options. Is it possible for the respondent to select several options in a question in the same time. Il seems he can select only one option in each question.

Please use the "Multiple Choice" or "Check Box" field. In this way, your users can select more than 1 option in a single Multiple Choice option
